Pre-K/Kindergarten Enrollment

Sequoyah Families,

In this unprecedented time in our nation’s history, school buildings are closed but school operations continue as we prepare for the next school year. 

We are excited to present an online Pre-K and Kindergarten enrollment form that parents/guardians can submit for the 2020-2021 school year.  Please follow the instructions listed below to enroll for Pre-K and Kindergarten.   This will be the initial portion of enrollment with an in-person session at a later date to complete the enrollment process once CDC guidelines allow.

PLEASE FOLLOW THESE INSTRUCTIONS TO MAKE SURE YOUR ENROLLMENT FORM IS COMPLETE:

1. Click on the link below to access the Sequoyah Elementary Pre/K and Kindergarten Enrollment form. 
2. YOU MUST DOWNLOAD  IT TO YOUR COMPUTER BEFORE FILLING IT OUT. 
3. In the upper right hand corner of the form you will see an arrow that points downward.  Click on it to download your form.
4. After it is downloaded to your computer, IT IS VERY IMPORTANT THAT YOU FIND IT ON YOUR COMPUTER AND OPEN IT.
5. Fill out the form and SAVE it using the  image in the top right corner of your form.
6. When you are ready to submit the form, use the information below to attach and send your file.
