Sports Physical Information

Deadline is July 15

As a result of the pandemic, we will be unable to have our yearly sports physicals that are normally held on campus.  Student athletes will need to complete a physical through their primary care physician or urgent care.  Athletes must have a physical by July 15, 2020 to participate in fall activities (softball, football, cross country, volleyball, band).

A physical form has been added to this website under the "Athletic Info" tab.  The physical form is two pages in length.  One page should be completed by a parent or guardian, and the second page completed by the doctor administering the physical.

Also you must complete the following RankOne forms prior to participation.  RankOne forms can be found under the "Athletic Info" tab on this website.  You will need your student school ID number.
