New Student Enrollment
Sequoyah Public School New Student Enrollment
Tuesday, August 3, 2021 from 8-11 am and 12-3 pm
New Student Enrollment will take place at each school site:
Elementary (grades prek-5)
Middle School (grades 6-8)
High School/Mid High (grades 9-12 will enroll at Mid-High)
RESIDENT: PERSONALLY OWN/RENT A RESIDENCY IN DISTRICT
Students that reside in the district MUST bring the following documentation:
1. Birth Certificate
2. Immunization /Shot Record with up-to-date vaccinations
3. Proof of Residency-either the most recent electric bill in parent's name with address or current lease/housing contract
4. CDIB card or other tribal registration, if applicable for the student
5. Court documents, if applicable for the student

TRANSFER: LIVING OUTSIDE OF SEQUOYAH SCHOOL DISTRICT Students who live outside the district MUST also complete a transfer application at the time of enrollment. A request for transfer does not guarantee enrollment at Sequoyah Public School. Pre-K Students MUST be 4 years of age on or before September 1st, 2021, to qualify for Pre-K enrollment and September 1st, 2020 for kindergarten. Please bring the required documentation to complete enrollment. |
